Your first visit to Cache Valley Surgical Consultants establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Patient Information / Patient Portal Account

Having your information, including personal, insurance, and medical, in our system before your visit will help your visit go quickly.  It also helps to have your full medical history to assure we are treating you in the best way possible for your issues.   You will need to register for a Patient Portal Account.  Once you have the account set up, you can then fill in the appropriate information.  You can also use the Patient Portal Account to access your medical records.   Click the link below to go to the Patient Portal Account web page.  You can also download an app to have access on your mobile phone / tablet.

Financial Policy

We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details.

Services will never be denied strictly on the ability to pay. We do, however, request that patients who are paying cash to visit with us to discuss payment options. For elective surgeries we do require one half of our responsible amount be paid prior to surgery.

It is our office policy to collect your insurance co-pay for services rendered in the office at the time of service. We will be happy to bill your insurance company for you, if you wish. We participate with most insurance plans, including Medicare and Medicaid.

Prescription Refill Policy

The best method to obtain a surgically related prescription refill is for you or your pharmacist to call our office early in the day. Most prescriptions can be refilled over the phone by the office staff after they check with the Doctor. Otherwise, you may contact your primary physician for your regular prescriptions.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
